We need your dinner choices, please!

We’d be grateful if you could fill out this form to choose your dinner options for the evening. Balls Brothers have requested this information from each attendee, and we need a separate form completed for each guest. Please send this link to your guests so that we may also record their choices or fill in the form on their behalf; do make sure they mention your name in the ‘table host’ section.

If we don’t hear from you by Monday 14 January, the default menu will be a Smoked Salmon starter, Chicken Breast main and Chocolate Brownie for dessert (or Tomato Basil Soup, Italian Tagliatelle and Chocolate Brownie if you have already specified you are vegetarian). It would help us if you could fill in the form, even if these are your choices.
